An incident review should help a team reduce the chance or impact of similar failures. Blameless does not mean avoiding accountability; it means examining the system, information, and conditions that shaped decisions rather than stopping at who was involved.

Build a factual timeline

  • Record when impact began, how it was detected, and when service recovered.
  • Separate confirmed observations from hypotheses that still need validation.
  • Describe customer impact at the level permitted by the incident's access rules.

Turn learning into owned work

For each follow-up, name an owner, a due date or review point, and the failure mode it addresses. Prefer changes that improve detection, containment, or recovery, not only reminders to be more careful.

Share the review with the people who can act on it and protect sensitive operational details. Revisit actions later to confirm that they changed the system as intended.
